HIPAA Privacy Policy
Vantage Health Collective respects the privacy of your health information. We support the protection of patient information through the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and other applicable privacy laws.
This statement provides general information about our approach to health information privacy. Your treating clinic’s Notice of Privacy Practices explains its specific legal responsibilities, permitted uses and disclosures, and your patient rights.
Your Care and Your Information
Clinical services are provided by the physician-owned practices operating at Vantage locations. Each practice is responsible for its patient records and applicable healthcare privacy obligations.
Vantage Health Collective provides nonclinical business and administrative support. When we handle protected health information on behalf of a practice, we do so under applicable agreements and legal requirements.
A shared Vantage brand does not give every location unrestricted access to your medical information.
How Health Information May Be Used
As permitted by law, your treating practice may use or disclose health information to:
-
Provide treatment and coordinate care with other healthcare professionals.
-
Verify insurance, obtain authorizations, submit claims, and receive payment.
-
Support practice operations, quality improvement, compliance, and administration.
-
Meet public health, oversight, and other legal obligations.
Certain uses and disclosures require your written authorization. Additional protections may apply to substance use disorder records, psychotherapy notes, and other sensitive information.
See your clinic’s Notice of Privacy Practices for a fuller explanation.
Protecting Patient Information
Our privacy and security requirements address authorized access, appropriate information sharing, workforce responsibilities, and safeguards for patient information.
Service providers handling protected health information on behalf of a practice must meet applicable contractual and legal requirements. This may include a business associate agreement.
No electronic communication or storage method can be guaranteed completely secure. Please use the communication methods recommended by your clinic when sharing sensitive information.
Your Privacy Rights
Subject to applicable law, you may have the right to:
-
Inspect or obtain a copy of your medical records.
-
Request corrections to inaccurate or incomplete information.
-
Request confidential communications.
-
Request restrictions on certain uses or disclosures.
-
Receive an accounting of certain disclosures.
-
Obtain a copy of your clinic’s Notice of Privacy Practices.
-
File a privacy complaint without retaliation.
Contact your treating clinic to exercise these rights. Our Privacy Contact can help direct your request to the appropriate practice.
Website Forms and Electronic Communications
General website forms and partnership inquiry forms are intended for basic contact information and administrative questions.
Please do not submit medical records, wound photographs, Social Security numbers, or detailed health information through these forms or ordinary email.
Contact your clinic for an appropriate way to provide medical information.
Submitting a website inquiry does not establish a provider-patient relationship. Website forms are not intended for emergencies. For a medical emergency, call 911.
